About The Position

Care Management for At Risk Children is a care management program for children, birth-to-five years of age, who may have long-term medical conditions, be in long-term stressful situations (i.e. have been exposed to adverse childhood experiences or adversely affected by social determinants of health), have an Infant Plan of Safe Care, and/or were in a Neonatal Intensive Care Unit (NICU). CMARC promotes the medical home model, linkage to community resources, provides support to families, and addresses barriers affecting the child’s care and health, such as social determinants of health. We are currently seeking a Nurse Supervisor to become part of this team! The Nurse Supervisor - Care Management for At Risk Children provides supervision to CMARC nurses and social workers to ensure the provision of quality patient-centered care to children, 0-5 years of age. This includes screening referrals from Pediatric Providers, Prepaid Health Plans, and other care/service providers; selecting and hiring qualified applicants; conducting regular supervisory conferences and annual performance reviews; and coordinating and collaborating with internal and external partners. CMARC is a program designed to promote health and wellbeing for young children through encouraging the use of the medical home, linking children and families to community resources and providing education and family support.

Requirements

  • Bachelor's degree in Nursing
  • Two years of experience as a nurse
  • North Carolina Board of Nursing Licensed Registered Nurse
  • CPR
  • AED
  • Equivalent education and experience are accepted

Nice To Haves

  • Bachelor's or Master's degree in Nursing
  • Two-five years of experience as a nurse working with NICU, Medically Complex Pediatric Needs, and Chronic Pediatric Conditions (Developmental and/or Medical)
  • Three-five years of supervisory experience
  • Completion of North Carolina Credentialed Public Health Nurse (NCCPHN) Curriculum
  • Ability to develop and maintain effective relationships with others in order to encourage and support communication and teamwork
  • Ability to build and maintain ongoing, collaborative, working relationships with coworkers to achieve the goals of the work unit
  • Ability to use appropriate methods to identify opportunities, implement solutions, and measure impact

Responsibilities

  • Integrate new staff into the work setting
  • Conduct required supervisory conferences and as needed
  • Conduct record reviews, assign referrals, monitor workflow and work assignments
  • Plan and conduct monthly staff meetings and provide updates of state and county guidelines
  • Provide continuing education and resource information
  • Provide guidance to staff working with families who may have priority medical and psychosocial risk factors
  • Monitor program performance by utilizing quality improvement tools and data reports to help ensure performance measures are met
  • Collaborate with health plans regarding performance measures and reports pertaining to members identified as priority
