About The Position

This position is funded by a grant for a duration of time as defined by the grant. Performs assigned professional nursing duties under the supervision of a higher-level nurse to support the Community Health Access & Navigation in Tennessee (CHANT) program, ensuring clients’ medical and non-medical needs are met.

Requirements

  • Three (3) years of nursing experience in a lead or supervisory capacity, and a bachelor's degree from an accredited college or university in nursing; OR
  • One (1) year of nursing experience in a lead or supervisory capacity, and a master’s degree from an accredited college or university in nursing.
  • PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED.
  • Must possess a valid Tennessee driver's license or secure one upon date of employment.
  • Must have daily access to an operable automobile and meet county requirements for automobile insurance upon date of employment.
  • Must be licensed or eligible for licensure in the State of Tennessee (licensure application must have been submitted upon date of employment).
  • Must maintain all required licenses, certifications, and training credentials throughout the duration of employment.
  • Basic physical assessment course must be completed within six (6) months of hire.
  • All employees hired after September 1, 1986, must be residents of Shelby County and shall continue to reside in the County as a condition of their employment.

Responsibilities

  • Reviews, approves, submits, and tracks all billable claims; maintains billing records and resolves denials to ensure accurate reimbursement.
  • Oversee pharmacy, treatment plan claims and other related claims to ensure weekly, accurate billing in coordination with nursing staff.
  • Manages CHANT program operations, including patient care priorities, timelines, outcomes, and grant deliverables to ensure quality, compliance, and timely service delivery.
  • Conducts scheduled and as-needed internal audits (e.g., Children with Special Services), collaborates with staff to address findings to ensure accuracy and compliance with state guidelines.
  • Manages fiscal activities, including monitoring expenditures, documenting activities, and ensuring financial compliance with program and grant requirements.
  • Prepares and submits all required state-mandated and internal reports, providing updates on program activities, productivity, and compliance issues
  • Overseas and monitors newborn screening data received from the state to identify potential enrollments and ensure appropriate follow-up.
  • Serves as a liaison to internal and external partners, coordinating communication related to billing requirements, documentation, deadlines, and program collaboration.
  • Provides education and support to patients, families, and staff through program guidance, orientations, in-services, trainings, and community or partner meetings.
  • Performs other related duties as required or directed.
