About The Position

Performs work promoting and protecting health of individuals, families, and communities through teaching, counseling, and applying preventative and rehabilitative measures, providing vaccines to individuals at county clinics, providing direct health care, education, and consultation to members of the community, responding to public health emergencies in a variety of setting, serving as back-up for staff positions, and related work as apparent or assigned. Work is performed under the direction of the Public Health Supervisor.

Requirements

  • Bachelor's degree in nursing, and minimal experience in home and community-based nursing, or equivalent combination of education and experience.
  • Minnesota Board of Nursing Registered Nurse on hire.
  • Minnesota Board of Nursing Public Health Nurse Certificate.
  • CPR (any certified person to teach CPR) on hire.
  • Must possess and maintain a valid driver’s license and proof of insurance.
  • Knowledge of nursing methods and techniques and how each is applied to the physiological, psychosocial, and spiritual functioning of the individual and family.
  • Knowledge of the functions of Federal, State, Health Care Plans, and other health care and social agencies and their relationships to public health programs.
  • Knowledge of billing practices of the health care plans.
  • Skill in assessing client's comprehensive needs and implement and evaluate nursing care plans, which use a broad array of community services.
  • Skill in the use of personal computers, associated software packages, hardware, and peripheral equipment.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to keep detailed records and make written and oral reports and generate computer reports.
  • Ability to make independent decisions, some with life-or-death implications.
  • Ability to accept supervision.
  • Ability to provide leadership and stimulate creativity in the development of programs and in services.
  • Ability to evaluate services provided and identify other appropriate services and/or develop new services.
  • Ability to respond to public health emergencies.
  • Ability to recognize and avoid hazardous situations such as minimizing exposure to body fluids, chemicals (ex. Methamphetamine), carcinogens, and secondhand smoke.
  • Ability to maintain all licenses and certifications required for the position.
  • Ability to establish and maintain effective working relationships with associates, clients, client families, public and private health care agencies, social workers, school personnel, and the public.
  • Knowledge of Health Insurance Portability and Accountability Act (HIPAA).
  • Ability to maintain a regular and timely work attendance.

Responsibilities

  • Coordinates a variety of community health programs such as: Disease Prevention and Control Child and Teen Child-Outreach Children's Dental services Early Hearing Detection and Intervention Birth Defects Elevated Blood Lead Perinatal Hep B Women, Infant and Children (WIC) Nutrition Program Family Home Visiting Programs: Maternal Early Childhood Sustaining Home Visiting Program (MECSH) Temporary Assistance for Needy Families (TANF) Maternal Child Health (MCH) Very Best Baby (VBB) Teenage Parenting Program (TAPP) Follow Along Program Pregnancy Testing Support of Aging and Disability Unit to provide nursing assessment
  • Assists with grant and budget management of programs.
  • Provides direction for Health Services Programs as needed or required.
  • Consults and cooperates with school and community personnel in promoting health programs.
  • Assesses individual and family needs through assessments completed by the nurse.
  • Develops and implements plans of care for the individual or family.
  • Assists in all phases of planning, detecting, controlling, and responding to viral pandemic and other health emergencies potentially affecting the public.
  • Provides education and referrals to community resources.
  • Performs other related duties as assigned.
