About The Position

Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community such as 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults as well as outpatient services such as medication management, case management, and counseling. Job Summary The FACT Program is a multi-disciplinary, clinical team that assumes responsibility for directly providing needed treatment, rehabilitation, and support services to adult persons with severe and persistent mental illness promoting recovery, empowerment, self-determination, and decision-making for each FACT participant. Some of the responsibilities of the FACT team include providing clinical and supportive services through advocacy, crisis intervention, assessments, evaluations, treatment planning, case management, and medication management. Participants are also provided with community referrals, symptom education, work-related vocational and educational services, social and interpersonal relationship training, and support for the participants’ families and caregivers. Most of a FACT team member's day is spent in the community completing many of the above duties. Only about 15% of their time is spent in the office. Company vehicles are available, however, members of the FACT team also need to use their personal vehicles to provide services and are eligible for reimbursement for mileage.

Requirements

In order to be considered, a candidate must have a Master's Degree in Nursing and current dual APRN Florida State Nursing license required.
ANCC certification in Psychiatric Nursing required.
DEA Certification required.
One year experience in psychiatric nursing with working with individuals with severe and persistent mental illness required.
Must demonstrate proficiency in the MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.
Strong communication skills are essential and this individual must be able to interact appropriately with internal and external customers, including patients, families, caregivers, community service providers, supervisory staff and other department professionals.

Responsibilities

Conducts psychiatric assessments including psychiatric history, course of illness, response to treatment, mental status examination, and DSM-V/ICD-10 diagnoses assists the team in determining the best treatment approach, with input from the individual and the family.
Provides medication management and oversees the medication self-administration program for all persons served by the FACT program to ensure symptom reduction and stability of persons served. Works closely with the FACT team and utilizes treatment strategies which include: assessing each person’s behaviors and symptoms of mental illness and prescribing medication in response to the symptoms; regularly reviewing and documenting the person’s symptoms of mental illness as well as his or her response to the prescribed medication treatment; educating the person regarding the symptoms of his/her illness and the effects and side effects of the medication prescribed to regulate it; assisting the person to identify the symptoms and occurrence patterns and to develop internal, behavioral, or adaptive methods to help lessen their effects; monitoring, treating, and documenting any medication side effects; developing and maintaining a positive, therapeutic relationship with the person served; using new atypical medications with greater effectiveness and a better side effect profile; and simplifying the medication regimen.
Completes screenings, assessments, and/or regular visits in both office and field-based settings as identified on the individual’s treatment plan. This may include visits to hospitals, CSUs, jails, or other community living environments.
Attends and participates in the team’s daily organizational meetings to assess patient status and progress, to coordinate treatment activities, and to develop treatment solutions to identified problems.
Coordinates, with the nurses, the medical treatment of all individuals enrolled in the program and consults with both staff and other agencies regarding diagnostic, medical, and treatment problems of persons served.
Provides accordance with State Regulations, MHRC Policies, Procedures and Standards, and the Program for Assertive Community Treatment (PACT) manual.
Each employee contributes to the completeness and confidentiality of clinical records by ensuring documentation, paperwork and system entries meet internal and external guidelines for content, accuracy and timeliness.
