L’Chaim Adult Day Health Center is a licensed facility and outpatient center that provides medical, social, and therapy services for Russian-speaking older adults and individuals with disabilities. We have been a haven of person-centered care and community support for thousands of emigre clients from San Francisco and San Mateo for over 25 years. POSITION SUMMARY: Under the supervision of the L'Chaim Supervisor Nurse I/Educator, the Psychiatric Nurse/On-Call RN participates as a member of the multidisciplinary team when indicated by participant needs, conducts behavioral health assessments, develops and provides care plan services, makes referrals, and provides nursing and personal care services on an “on-call” basis. Additionally, the Psychiatric RN provides consultation to center staff three hours per month per Title 22, Section 54325.

Requirements

  • Licensed by the California Board of Registered Nursing
  • Certified Psychiatric Nurse
  • Current CPR and First Aid certification
  • Experience working with the elderly or chronically ill required
  • Ability to communicate appropriately and effectively with participants, professional staff, families, and caregivers
  • Ability to effectively utilize electronic health record software, Microsoft Office Suite software including Outlook

Nice To Haves

  • Experience working in an adult day health care or skilled nursing facility preferred
  • Russian language conversational skills preferred

Responsibilities

  • Conducts initial assessment and subsequent reassessments (at a minimum of every six months) of each participant with a psychiatric diagnosis or medical history that indicates a mental, emotional, or behavioral problem
  • Ensures that behavioral health services provided meet the requirements specified in Title 22, California Code of Regulations, Sections 54325, which include participant assessments and reassessments, individual plan of care, and supervised plan of care
  • Coordinates implementation of behavioral health services plan of care with Center Nurse and MSW
  • Prepares and provides individual plans of care (IPC) for participants requiring psychiatric services
  • Provides behavioral health education and counseling to the participants and their families, when indicated in their plans of care
  • The psychiatric nurse will work the hours necessary to meet the needs of the participants based on their care plans and to meet the requirements specified in Title 22, California Code of Regulations, and Sections 54325
  • Provides consultation to center staff three hours per month per Title 22, Section 54325
  • Participates in multidisciplinary team (MDT) meetings if psychiatric services are provided or are recommended by the physician; May serve as a member of the Utilization Review Committee
