As a PMHNP at Access Healthcare Associates, you’ll join a purpose-driven team that delivers continuum-based care, integrating psychiatric services with primary care and other supportive specialties, operates within a collaborative culture, values stability, partnership, and mentorship, balances clinical excellence and efficiency with compassion, presence, and genuine human connection, and supports provider autonomy through structured onboarding, clear workflows, and dedicated field-based support. This role offers a geriatric psychiatric focus, mobile, relationship-based care in home and senior living settings, autonomy with support from a dedicated medical assistant and medical scribe, and a focus on quality outcomes, continuity, and provider satisfaction.
Requirements
- NP/PA: Current, active, and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in good standing in the state of practice required and must be maintained throughout employment.
- Strong clinical judgment, communication skills, and comfort working independently with remote support.
- A compassionate, patient-centered approach with respect for seniors and their families.
Nice To Haves
- Experience in geriatric psychiatry, long-term care, or mobile healthcare preferred.
Responsibilities
- Conducting comprehensive psychiatric evaluations for geriatric patients in home and facility-based settings.
- Diagnosing and managing mood disorders, anxiety disorders, dementia-related behavioral symptoms, and other psychiatric conditions.
- Prescribing and managing psychotropic medications with careful consideration of comorbidities and geriatric physiology.
- Providing crisis intervention, behavioral guidance, and family education when clinically indicated.
- Collaborating with primary care providers, therapists, and facility care teams to ensure coordinated, whole-person care.
- Building trusted relationships with patients, caregivers, families, and community partners.
- Maintaining consistent, accurate clinical documentation and follow-through.
