About The Position

The Program Director will oversee day-to-day implementation of the RISE@JASA initiative at Casa Celina (1001 Thieriot) and supervise the Residential Service Coordinators at 1490 Southern Blvd and Casa Celina. This includes enhancing person-led care coordination, supporting adaptation of the RISE model for a housing-based context, leading staff training and supervision, and collaborating with partners to monitor program fidelity and impact. The Director will serve as a liaison between JASA, the RISE team, housing staff, and community-based service providers to ensure coordinated, trauma-informed support for residents at risk of EASN. Provide supervisory oversight of the case management and on-site programming services rendered by Residential Services at Casa Celina and 1490 Southern Blvd. Maintain oversight and monitor the status of all cases identified as elevated risk. Monitoring of staff and case review with regard to ongoing reassessment of client need, monitoring of service efficacy, and appropriate revision of the case plan. Informing Senior Leadership of emerging issues and/or crisis situations with clients; work with Residential Services staff to develop a plan to resolve the risk. Assist with home visits as identified.  Supervise MSW interns if applicable. Complete required case records, reports and statistics, utilizing computers, if so required.   Serve as the primary on-site lead for the RISE@JASA project at Casa Celina which includes overseeing the integration of person-led care coordination and the RISE elder justice model into housing-based workflows and interdisciplinary team (IDT) meetings. Serve as the primary liaison with the RISE team for training, technical assistance, and evaluation. Support implementation of the mixed-methods evaluation in partnership with the RISE team, including data collection and reporting.   1. Facilitate monthly interdisciplinary team (IDT) meetings and ensure effective communication across JASA’s mental health, case management, APS, and housing teams. 2. Support data and evaluation activities to ensure accurate tracking of service delivery and resident outcomes. Use data to inform program improvements and contribute to dissemination materials. Document lessons learned and support internal and external dissemination of best practices. 3. Collaborate with Residential Service Coordinators to deliver coordinated, goal-driven, trauma-informed care for older adults. 4. Participate in RISE training and provide ongoing coaching and reflective supervision to Casa Celina staff. Ensure fidelity to RISE modalities, including motivational interviewing, supported decision-making, restorative practices, and interdisciplinary teaming. 5. Coordinate with JASA leadership to resolve barriers and align with agency goals.

Requirements

  • Master’s degree or candidate for Master’s degree in social work, public health, gerontology, or related field required.
  • Minimum 5 years of relevant experience, including program leadership, care coordination, or housing-based services.
  • Supervisory experience and commitment to staff development.
  • Strong organizational, communication, and problem-solving skills.
  • Ability to lead in a collaborative, interdisciplinary environment.
  • Familiarity with data tracking, program evaluation, and reporting.

Nice To Haves

  • Demonstrated experience with trauma-informed care, elder justice, or vulnerable populations strongly preferred.
