About The Position

The role of the Forensic Services Support Program Case Manager is to provide case management services to incarcerated individuals who experience mental and behavior health concerns. The Case Manager will support the individuals, during and post incarceration, to ensure a stable transition back into the community. This would occur by employing a holistic service/strengths-based approach that addresses conditions within the offender’s life that could contribute to recidivism, joblessness, homelessness, and/or substance abuse relapse. The Forensic Services Support Program Case Manager will be an advocate for individuals and assure there are effective resources to support the individual. Glade Run recognizes that maintaining service continuity as the individual moves through the criminal justice system and returns to the community is critical.

Requirements

  • Bachelor’s Degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling or education.
  • One (1) to three (3) years’ experience working with adults in a mental health^ setting or persons involved in the criminal justice system.
  • One (1) to three (3) years of case management experience.
  • Must possess excellent verbal and written communication skills
  • Must have a valid driver’s license, acceptable driving record, and access to a reliable automobile
  • Ability to adapt hours to needs of position.
  • Access to a telephone.
  • Ability to establish and maintain working relationships with agency personnel and community stakeholders
  • Must be comfortable working in a correctional facility environment
  • Must have emotional stability to perform essential job functions.
  • Must be able to work quickly and effectively
  • Must be able to be able to work in a flexible environment
  • Ability to remain organized despite pressured situations.
  • Must have acceptable PA State Police clearance, PA Child Abuse History Clearance, and FBI Clearance

Responsibilities

  • Establish rapport with the client to provide orientation in regards to how services are delivered including frequency of services, how to communicate with case managers, and crisis planning.
  • Complete the Psychosocial and Health Assessment to obtain pertinent social, psychiatric, developmental, family, educational, medical and other data essential for planning the treatment for each individual serviced.
  • Develop a Service plan for the client to identify appropriate resources and supports. Review the Service Plan with clients on a weekly basis to ensure they are receiving the necessary support to aid in their recovery.
  • Link clients returning to the community with the appropriate support by looking at the individual’s strengths and current needs that were identified in their service plan and then facilitating connections with those resources prior to their release.
  • Monitor progress of clients. Engage with individuals on a consistent basis fostering a supportive, therapeutic relationship and will meet the individuals wherever they are at to support them. Coach and support the individual to expand their comfort levels in other settings to assist them with connecting to other community supports.
  • Work cooperatively as part of a team to provide individuals with the best treatment possible including collaborating closely with the County Forensic Services Manager, the criminal justice system, the county jail staff, treatment members of the FSSP, and other community resources.
  • Provide reports/testimony regarding progress, support and encourage individuals in adhering to any court sanctioned follow up/through, and assist individuals with advocating for themselves as well as providing advocacy as needed.
  • Complete all required documentation in an accurate and timely manner.
