About The Position
We are seeking a compassionate and skilled PRN Social Worker to provide psychosocial support and care coordination services on an as-needed basis. This role focuses on assessing patient and family needs, connecting individuals with appropriate resources, and supporting safe, effective transitions of care.
Requirements
- Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program
- Current state Social Work license or certification in good standing (as required by role)
- Knowledge of community resources, healthcare systems, and discharge planning processes
- Strong communication, organizational, and problem-solving skills
- Ability to work independently and manage a variable caseload
Nice To Haves
- Prior experience in a healthcare or clinical setting (e.g., hospital, rehabilitation, SNF, home health)
- Experience with electronic medical records (EMR) systems
- Case management or care coordination experience
- Flexible availability, including weekdays, weekends, or holidays as needed
Responsibilities
- Conduct psychosocial assessments to identify patient, family, and social support needs
- Provide counseling, emotional support, and crisis intervention within scope of practice
- Assist with discharge planning, care coordination, and referrals to community resources
- Advocate for patients and families to ensure access to appropriate services
- Collaborate with physicians, nurses, therapists, and interdisciplinary team members
- Document assessments, interventions, and care plans accurately and in a timely manner
- Ensure compliance with ethical standards, regulatory requirements, and organizational policies
- Participate in care conferences and interdisciplinary meetings as needed
