As a nurse practitioner you will: • Partner with local communities and senior living organizations to build our presence • Help shape market strategy and patient panel development • Influence clinical workflows and care practices as the market grows • Build trusted relationships within care ecosystems across St. Louis • Serve as a key ambassador for Bloom in one of our newest territories

Requirements

  • Active or eligible Missouri license
  • Passion for high-touch, relationship-centered primary care
  • Interest in building a new market and shaping growth
  • Comfort working autonomously within a high-collaboration team model
  • Curiosity, initiative, and a builder’s mindset

Responsibilities

  • Deliver comprehensive primary care through at-home and senior living visits
  • Manage an empaneled patient population and provide approximately 10 visits/day
  • Collaborate with an interdisciplinary team including specialists, pharmacists, social workers, and care managers
  • Use population health insights to support better outcomes
  • Participate in ongoing training and development (including occasional travel to Denver)
  • Build strong partnerships with senior living leaders, hospital systems, and community organizations
