Life Medical is a mobile care practice dedicated to delivering compassionate, high-quality primary care directly to patients in the comfort of their homes. We serve individuals who are unable to travel to traditional medical offices due to age, disability, or chronic illness. Our mission is to improve access, reduce barriers, and provide personalized care where it’s needed most. Role OverviewWe are seeking a dedicated and autonomous Primary Care Nurse Practitioner to join our growing team. In this role, you’ll conduct in-home visits, manage chronic and acute conditions, and build meaningful relationships with patients and their families. You’ll be part of a supportive, mission-driven practice that values flexibility, independence, and clinical excellence.
Requirements
- Licensed Nurse Practitioner in the state of New Jersey
- Board-certified in Family or Adult-Gerontology Primary Care
- Minimum of 1 year of clinical experience (home care experience preferred)
- Strong clinical judgment and ability to work independently
- Excellent communication and interpersonal skills
- Valid driver’s license and reliable transportation

Responsibilities
- Provide comprehensive primary care services during in-home visits
- Perform physical exams, diagnostic assessments, and routine screenings
- Manage chronic conditions and acute illnesses
- Prescribe medications and develop individualized care plans
- Coordinate with other healthcare providers and community resources
- Document visits and maintain accurate medical records
- Educate patients and caregivers on health management and preventive care
