About The Position

The Prevention Case Manager Supervisor (PCMS) provides direct supervision to the Prevention Case Manager team while also carrying a caseload of participants at risk of homelessness. This position ensures high-quality delivery of housing-focused case management services to families at risk of homelessness, primarily through partnerships with schools and churches. The PCMS role requires strong judgment, adaptability, and the ability to monitor program fidelity, and partners with internal and external stakeholders to promote housing stability and holistic support for individuals and families.

Requirements

  • Bachelor’s degree in Social Work, Human Services, or related field required; Master’s degree preferred.
  • Minimum 2 years of case management experience in housing or homelessness prevention services.
  • At least 1 year of supervisory, lead, or mentorship experience.
  • Experience in school, church, or family-centered service environments preferred.
  • Bilingual in English and Spanish preferred.
  • Must meet all background check requirements applicable to working within any Colorado school district.
  • Must possess a valid driver’s license and be able to travel frequently, typically between Denver and Northern Colorado on a weekly basis or more often, with occasional travel to other locations as required.
  • Excellent written and verbal communication skills.
  • Strong time management, organizational skills, and ability to prioritize independently.
  • High attention to detail, particularly in documentation and compliance.
  • Ability to engage professionally with families under financial and emotional stress.
  • Flexibility to adapt to varied work schedules, including occasional early mornings or evenings.
  • Demonstrated ability to work independently in the field while maintaining close collaboration with internal teams.
  • Must be a Christian who affirms the beliefs set forth in the Statement of Faith, demonstrated through personal testimony and Christian conduct.

Responsibilities

  • Initiates and maintains effective working relationships with DRM departments and external partners.
  • Represents DRM professionally in all interactions with clients, schools, churches, and other community contacts.
  • Participates in required meetings, trainings, and organizational events.
  • Monitor team caseloads, service plans, and documentation to ensure quality and compliance.
  • Provides on-boarding and training for new Case Managers and Interns.
  • Represent the Prevention team in multidisciplinary meetings, community collaborations, and case conferences.
  • Assists the Director in program and staff development projects.
  • Assists the Director in identifying and establishing partnerships with outside agencies.
  • Represents Denver Rescue Mission at donor events, community events, etc. as scheduled.
  • Support community engagement and outreach efforts as directed.
  • Support preparation for audits, reports, and data tracking related to program performance and funding sources.
  • Identify trends, barriers, and service gaps, and make recommendations for improvements to service delivery
  • Manage a dynamic caseload of participants at imminent risk of eviction or housing loss, including school and church-referred clients.
  • Provide timely, housing-focused case management, including assessments, service planning, resource navigation, and eligibility determination for prevention services.
  • Assist participants in accessing public benefits, rental assistance, and community-based supports that promote short-term housing stability.
  • Collaborate with families to develop individualized plans focused on budgeting, employment retention, and long-term stability.
  • Maintain accurate documentation in HMIS and internal systems, including case notes, eligibility assessments, and service outcomes.
  • Provide culturally responsive, equitable support to families of diverse backgrounds, cultures, and beliefs.
  • Participate in ongoing professional development and training activities.
  • Perform other duties as assigned.
