Requirements

  • Education: High School Diploma.
  • Experience: Care coordination, case management, or a related field.
  • Skills: Strong organizational, communication, and interpersonal skills.
  • Ability to work collaboratively with a multidisciplinary team.

Nice To Haves

  • Experience working within a Preferred Provider network is highly desirable.
  • Certifications: Relevant certifications, such as Certified Case Manager (CCM) or Certified Professional in Healthcare Quality (CPHQ), are a plus.

Responsibilities

  • Patient Care Coordination: Manage and coordinate patient care across various Preferred Provider settings to ensure seamless and efficient care delivery.
  • Serve as the primary liaison between the Preferred Provider network and Pinnacle Home Care.
  • Compliance: Ensure all activities comply with Preferred Provider guidelines and regulations.
  • Care Plan Development: Collaborate with Preferred Provider professionals to develop and implement individualized care plans for patients.
  • Patient Education: Educate patients and their families about their conditions, treatment options, and care plans.
  • Data Management: Maintain accurate and up-to-date patient records and documentation.
  • Quality Improvement: Identify opportunities for quality improvement and implement strategies to enhance patient outcomes.
  • Communication: Serve as the primary point of contact for patients, families, and healthcare providers within the Preferred Provider network.
  • Resource Coordination: Connect patients with community resources and support services as needed.
  • Reporting: Prepare and present reports on patient outcomes, care coordination activities, and compliance with Preferred Provider guidelines.
