Description Job Description Oversees scheduling, assigns daily duties, and provides guidance and direction to the nursing assistants. Job Responsibility Collaborates with administrative and medical staff leadership in defining challenges, opportunities, and initiatives. Works to enhance outreach and education efforts within the community. Coordinates and organizes internal and external events including developing and implementing an itinerary; scheduling of staff and volunteers; securing available resources, supplies, and locations; ensuring coordination of all involved. Collaborates with the clinical team to determine treatment plans for patients, and reviews and documents evaluations, assessments, diagnostic testing, and other appropriate data. Participates in quality meetings to discuss cases and review clinical outcomes. Organizes clinical pathways to promote quality patient care and optimize outcomes. Ensures follow-up communications are carried out with patients post discharge. Develops patient-centered transition plan to supplement discharge plan created. Meets the educational needs of families and other health care professionals concerning the care of the patients. Performs related duties as required. Job Qualification Master’s Degree in a Nurse Practitioner Program and National Board Certification in specialty area required. Current license to practice as a Nurse Practitioner in New York State, and current license to practice as a Registered Professional Nurse in New York State required. OR Bachelor’s Degree, required. Graduate of an accredited (Accreditation Review Committee for Physician Assistant Programs ARC-PA) program required. Current license to practice as a Physician Assistant (PA) by the New York State Department of Education in New York State, or current NCCPA Certification from the National Commission on the Certification of Physician Assistants (NCCPA) required. OR Master’s Degree in clinical nursing practice. Requirements

  • Master’s Degree in a Nurse Practitioner Program and National Board Certification in specialty area required.
