About The Position

The PRE-Transition Specialist is responsible for using sound professional judgement and best practices to ensure quality case management services necessary to assist youth who are transitioning out of foster care in achieving their goals.

Requirements

  • Required – Associate’s degree or 60 college hours in social services or a related field.
  • Required – 1 year of full-time paid experience working with Youth/Young Adults related to housing, education, and employment
  • Maintain certificates required for the position including but not limited to First Aid and CPR.
  • Valid Texas Driver’s license with a clear driving record and reliable transportation.
  • Pass a pre-employment drug screen and random drug screens.
  • Provide proof or work eligibility status upon request.
  • Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).

Responsibilities

  • Access personal needs of PAL clients and help to obtain resources to meet those needs by providing case management services.
  • Advocate with human services, health, and court and criminal justice systems to resolve issues facing clients.
  • Assists youth in crisis by providing de-escalation for situation and developing plan with youth to move out of state crises.
  • Develop detailed Individual Service Plans for each client and meet with them regularly to assess progress and update the plans.
  • Maintain timely and accurate file documentation and data entry that meet the requirements of the PAL program.
  • Follow all DFPS guidelines and policies concerning PAL TFSS services.
  • Demonstrate a working knowledge of all contracts and agency policies and procedures.
  • Meet all federal and state regulatory guidelines and standards that are applicable to this position.
  • Participate/lead PAL program weekly, monthly events and activities.
  • Work evenings, weekends and holidays as needed or requested by position supervisor.
  • Implement BCFS safety protocols including evacuating with children and other staff in case of an emergency.
  • Maintain confidentiality in all areas of the service population and program operations.
  • Maintain BCFS HHS professional and ethical standards of conduct outlined in the employee handbook including demonstrating respect for agency staff, the service population, and community members and complying with the required dress code, at all times.
  • Field calls, emails, visitors daily.
  • Maintains front lobby with proper daily items.
  • Attend all required trainings as applicable.
  • Maintains operational office equipment and supplies, at all times.
  • Demonstrate the ability to: a. Respond sensitively and competently to the service population’s cultural and socio-economic characteristics. b. Communicate effectively in writing and verbally in English. c. Work in a fast-paced environment. d. Maintain computer literacy required to meet the responsibilities of the position. e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team. f. Travel as needed.
