About The Position

Position Summary Fedcap NYC provides programs and services that change the lives of marginalized people facing barriers to economic and social wellbeing. Our Transition Planning & Transportation program (TPT) provides comprehensive, culturally competent services to individuals detained in city jails and supports their successful reentry into the community. As Transition Specialist, you will provide assessment, planning, coordination, and delivery of evidence-based support, crisis intervention, and collaboration with internal and external stakeholders to ensure continuity of care. You will also ensure that each participant completes a transition plan and identifies goals for post-release services.

Requirements

  • You have a Bachelor’s degree from an accredited institution preferred; a minimum of Associates degree or equivalent credential required.
  • You bring a minimum of 2 years of experience in case management, workforce development, or social services OR in related fields, such as education, healthcare administration, or human resources.
  • You must pass the required security clearance with the NYC Department of Corrections to work inside Rikers Island

Responsibilities

  • Schedule individualized transition plan sessions to 90% of Participants on caseload from the point of incarceration
  • Coordinate referrals for post-release services with the Community Partnership Manager
  • Identify appropriateness to attend workshops for Participants engaging in services
  • Complete a comprehensive assessment of needs and develop housing referrals and re-entry supports for post-release engagement.
  • Provide Case Management services to 100% Participants engaging in services at least 1x/month
  • Complete transition plans with 80% participants to determine community service needs upon reentry.
  • Completing the LSCMI Assessment for high-level need participants with substance and mental health barriers
  • Coordinate with the Employment Specialist for career readiness checklists
  • Document 100% case management activities in the DOC Tracking and FedcapCARES within 48 hours of service
  • Monitor, discuss, and track improvement needs a minimum of 1X/month in partnership with the Department of Corrections
