The role of the Independent Living Transition Specialist and/or Youth Development Specialist is to support the planning, preparation and development the competencies of youth in the foster care system. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. 1) Conduct pre-independent living life skills assessments for eligible foster care youth age 17 by using established assessment tools. 2) Conduct aftercare/transitional services transition plans based on an assessment of need and identified goals for eligible young adults ages 18-23. 3) Conduct life skills training, ILS foster parent training and care management training as directed by ILS Coordinator. 4) Based on the outcomes of the IL assessment, develop a life skills training plan for each youth or young adult with a schedule of targeted Life Skills classes to develop competencies in identified areas. 7) Participate in case management permanency staffing's and IL transition staffing's for 17 year old youth. 8) Provide independent living life skills training updates to case management system of care as part of the youth's permanency staffing and judicial reviews. 9) Provide targeted aftercare services for 18 - 23 year olds who are receiving services through the transition, aftercare or road to independence program. 10) Maintain program documentation on assessments, training and services specific to the youth in FSFN. 11) Assist Independent Living Director in maintaining files, youth applications, assessments and life skills education plans in accordance with QA standards.
