About The Position

Transition Specialist Location: Florida: Lake Mary, Miami, Tampa. Virtual and travel/field visits. Field: This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement. The Transition Specialist is responsible for preparing members with Serious Mental Illness (SMI) or physical illness for their transition into independent community living pre and post-relocation. Assists members with facilitating independence in community living by providing education; identifying tools and resources to perform self-care using environment modifications, and engaging with community partners to build strategy with local stakeholders. Advocates on behalf of the member, helping resolve case or broader systems issues. Supports service coordination teams and connects members with those who can provide insight and expertise to assist in overcoming challenges.

Requirements

  • Requires a BS/BA in Health, Social Services or related field.
  • Previous experience in behavioral health, social work, or the transition of individuals from institutional settings into the community strongly preferred.
  • A valid state driver's license may be required.
  • Travels to worksite and other locations as necessary.

Nice To Haves

  • Experience in case management.
  • Bilingual in English and Spanish with strong documentation skills in English.
  • 2 years of related experience.
  • Registered nurse licensed in the state of Florida.

Responsibilities

  • Coordinates with the Service Coordinator and nursing facility to develop a transition plan.
  • Provides intensive psychosocial rehabilitative support, initiating therapeutic interventions as needed.
  • Conducts cognitive adaptation training and other evidence-based skills training for members.
  • Conducts evaluation assessment interviews with members regularly.
  • Performs periodic on-site visits with members as needed.
