Transition Specialist Location: Florida: Lake Mary, Miami, Tampa. Virtual and travel/field visits. Field: This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement. Please note that per our policy on hybrid/virtual work, candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment, unless an accommodation is granted as required by law. The Transition Specialist is responsible for preparing members with Serious Mental Illness (SMI) or physical illness for their transition into independent community living pre and post-relocation. Assists members with facilitating independence in community living by providing education; identifying tools and resources to perform self-care using environment modifications, and engaging with community partners to build strategy with local stakeholders. Advocates on behalf of the member, helping resolve case or broader systems issues. Supports service coordination teams and connects members with those who can provide insight and expertise to assist in overcoming challenges.
