The Pre-Release Care Manager provides person-centered, care coordination services to justice-involved Medi-Cal beneficiaries during the 90-day pre-release period in accordance with California CalAIM Justice-Involved Reentry requirements. This role is responsible for conducting Health Risk Assessments (HRAs), developing individualized Reentry Care Plans, coordinating the required “Mandated 7” pre-release services, and ensuring continuity of care through effective information exchange and warm handoffs to community providers and Managed Care Plans (MCPs). The Pre-Release Care Manager collaborates with interdisciplinary clinical teams, correctional staff, MCP care managers, and community-based organizations to ensure individuals are medically, behaviorally, and socially prepared for successful transition to the community upon release.

Requirements

  • Active California licensure that permits comprehensive assessment and care planning across medical and behavioral health needs (e.g., Registered Nurse, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, or Licensed Professional Clinical Counselor), as required by program scope.
  • Minimum of 2–3 years of experience in care coordination, case management, behavioral health, correctional healthcare, or a related healthcare setting.
  • Active CPR/BLS certification required if role is based in a correctional facility (American Heart Association or American Red Cross guidelines).

Nice To Haves

  • Experience working with justice-involved populations, Medicaid/Medi-Cal beneficiaries, or reentry programs strongly preferred.
  • Knowledge of CalAIM Justice-Involved Reentry requirements, Medi-Cal managed care coordination, and community resource systems preferred.
  • Experience coordinating multidisciplinary services, discharge planning, and transitions of care.

Responsibilities

  • Health Risk Assessment & Reentry Care Planning
  • Conduct comprehensive Health Risk Assessments (HRAs) to identify physical health, behavioral health, substance use, and social drivers of health (e.g., housing, food security, transportation, and community supports).
  • Develop individualized Reentry Care Plans that outline post-release appointments, medications, durable medical equipment (DME), and community resource linkages.
  • Update care plans based on changes in clinical status, release planning timelines, or identified needs.
  • Maintain accurate and timely documentation in the EMR and applicable Medi-Cal or MCP systems in accordance with regulatory and billing requirements.
  • Coordination of CalAIM “Mandated 7” Pre-Release Services
  • Coordinate and ensure delivery of all required pre-release services, including: Reentry Care Management Physical Health Clinical Consultation Behavioral Health Clinical Consultation Laboratory and Radiology Services Medications and Medication Administration Medication Assisted Treatment (MAT), as clinically indicated Warm Handoff to Community Providers and Services
  • Collaborate with internal clinical teams to ensure services are completed within required CalAIM timeframes prior to release.
  • Care Coordination & Transition Support
  • Coordinate post-release appointments with primary care, specialty care, behavioral health, and substance use treatment providers.
  • Ensure individuals are released with a 30-day supply of medications, required prescriptions, and necessary DME.
  • Facilitate warm handoffs to community providers, MCP care managers, and social service agencies to promote continuity of care.
  • Provide education on medication adherence, chronic disease management, behavioral health follow-up, and use of community-based services.
  • Information Exchange & Managed Care Plan Coordination
  • Obtain and document appropriate patient consent to share medical and care coordination information with Medi-Cal Managed Care Plans (MCPs) and community providers.
  • Coordinate with MCP care managers to align reentry care plans, referrals, and post-release service delivery.
  • Ensure timely exchange of clinical records, care plans, and discharge summaries in compliance with HIPAA and state regulations.
  • Resource Navigation & Social Drivers of Health Support
  • Identify and coordinate community resources addressing housing, food security, transportation, employment, and social support needs.
  • Provide individuals with written and verbal instructions regarding appointments, medications, and available reentry services at the time of release.
  • Assist with linkage to community-based organizations, peer support programs, and reentry services.
  • Collaboration & Communication
  • Work closely with nursing, behavioral health, mental health, pharmacy, and administrative teams to ensure coordinated service delivery.
  • Communicate regularly with correctional facility leadership, custody staff, and interdisciplinary teams regarding release planning and care coordination needs.
  • Participate in interdisciplinary case reviews, care coordination meetings, and quality improvement initiatives.
  • Compliance, Documentation & Reporting
  • Document all care management activities in accordance with Medi-Cal, CalAIM Justice-Involved Reentry, and facility requirements.
  • Ensure compliance with all regulatory requirements related to pre-release care management and continuity of care.
  • Track required service metrics, timelines, and outcome measures for program monitoring and reporting.
  • Maintain strict confidentiality and adhere to HIPAA, state regulations, and facility policies.
  • Quality Improvement & Program Support
  • Participate in audits, quality monitoring, and program evaluations related to Justice-Involved Reentry services.
  • Support workflow development, documentation accuracy, and compliance with CalAIM requirements.
  • Remain current on regulatory updates impacting Medi-Cal Justice-Involved initiatives and reentry care coordination.
  • Other Duties Support reentry team projects, training, and workflow improvements.
  • Perform other duties as assigned.
