About The Position

The Practice Coordinator supports the day-to-day operational, scheduling, and relational needs of behavioral health providers serving post-acute and Skilled Nursing Facility (SNF) settings. This role is a key operational liaison between Nurse Practitioners, therapists, facilities, and internal leadership to ensure smooth workflows, consistent provider coverage, and high-quality service delivery to facility partners. The ideal candidate is highly organized, detail-oriented, and relationship-driven, with experience in healthcare operations, scheduling, or practice coordination. This position plays a critical role in supporting provider efficiency, facility satisfaction, and overall practice growth.

Requirements

  • Minimum of 2–4 years of healthcare operations, practice coordination, scheduling, or administrative support experience.
  • Experience in behavioral health, long-term care, skilled nursing facilities, or post-acute care preferred.
  • Strong organizational and multitasking skills with the ability to manage multiple providers and facilities.
  • Excellent written and verbal communication skills with a professional, customer-service oriented approach.
  • Proficiency in Microsoft Outlook, Excel, Google Workspace, and scheduling tools.
  • Ability to work independently in a remote or hybrid environment with minimal supervision.

Nice To Haves

  • Experience supporting multi-state provider teams.
  • Familiarity with SNF workflows, GDRs, PCM meetings, and post-acute clinical operations.
  • Experience working with EMR systems and healthcare scheduling platforms.

Responsibilities

  • Serve as a primary point of contact for post-acute facilities regarding provider scheduling, coverage, and operational communication.
  • Coordinate communication between behavioral health providers (NPs, Psychologists, Therapists) and facility staff.
  • Send courtesy communications to facilities regarding provider absences, coverage changes, or schedule updates.
  • Maintain provider and facility contact lists and communication workflows.
  • Provider & Facility check-ins / 30-days, 60-days, 90-days with new employees and new facilities
  • New provider training schedules for first 30 days.
  • Assist with daily and weekly provider scheduling and rounding coordination.
  • Support coverage coordination when providers are out of office, including sending coverage assignment communications.
  • Track provider availability, PTO, and schedule changes in collaboration with clinical leadership.
  • Support onboarding logistics for new providers, including introductions to facilities and schedule setup.
  • Support preparation of patient and provider operational workflows (e.g., meeting schedules, rounding calendars, GDR schedules, PCM coordination).
  • Assist with preparation and distribution of operational reports and schedules to internal teams.
  • Track operational tasks, deadlines, and follow-ups to ensure timely execution.
  • Support internal operational initiatives and special projects as assigned.
  • Build and maintain strong relationships with facility partners and internal provider teams.
  • Respond promptly to facility and provider inquiries and escalate issues as needed.
  • Support leadership in maintaining high facility satisfaction and provider engagement.
  • Maintain organized operational records, schedules, and documentation.
  • Assist with onboarding documentation coordination (non-credentialing tasks).
  • Support compliance with internal policies and post-acute operational requirements.
