About The Position

The Population Health Specialist (PHS) is responsible for assisting the IWA/CCC participating primary care practices in achieving their goals in value-based programs. The PHS is a practice champion effectuating change in everyday practice procedures, an education resource for specialist and facility utilization, and assisting the IWA/CCC practices in meeting quality and efficiency goals. In working with IWA/CCC practices, this role will use data from payor sources, including data in the population health tool, practice EMRs and specific payor portals to identify individual patient needs relating to preventative screens, laboratory testing and general maintenance of wellness. The PHS is adept at a variety of office-based EMRs to maximize accuracy and efficiency around documenting care, entering value-based care metrics and closing care gaps. In particular, the PHS is proficient in all population health tools used within the IWA/CCC as well as other programs offered by various third party payors. The PHS works closely with representatives of the payors and attends monthly JOC meetings, proactively maintains regular contact with assigned IWA/CCC practices’ providers, population health champions, and key personnel through office visits, meetings, email and telephonic communication. The PHS is knowledgeable about existing and new Redeemer Health services and offerings and promotes them appropriately to facilitate in system/Tier 1 cost effective utilization. This role understands the importance of value-based health care and is able to engage providers, office staff, and patients, to achieve health improvement goals in defined populations served by IWA and the CCC. This role is responsible for guiding technology utilization (i.e., Office EMRs and Population Health Tools) for the advancement and optimization of value-based care performance in IWA/CCC practices. Requirements

  • Bachelor’s degree
  • Three years of experience in a health care setting to include Population Health experience
  • Proven skills in delivering a positive patient experience, knowledge of health care related terminology, critical thinking skills, proficient computer skills and have excellent communication (both written and oral) skills.
  • Impeccable organizational skills and the ability to work in and adjust to different environments.
  • Willingness to travel to each IWA/CCC practice and flexibility in scheduling visits to physician offices.
  • Comfortable with telephonic outreach to physicians, office personnel, and, patients, as well as scheduling, facilitating, and coordinating physician and patient testing appointments.
  • Proficient in Word and Excel
