About The Position

The Population Health Outreach Specialist supports Primary Care initiatives such as Value- Based Programs and Quality Incentive Programs across the organization. The Population Health Specialist will focus on the IHI Triple Aim goals which include improving overall patient experience, improve quality of care delivered to patients, and reducing cost of care delivered. The Population Health Specialist works as an integral member of the centralized quality team to identify and address high priority areas to maximize efficiencies to improve outcomes. Working collaboratively with cross functional business partners, the Population Health Specialist will gain a working knowledge of many areas of the organization and understand the dependencies and processes that impact program successes. The incumbent will work directly with the Director of Primary Care and the centralized quality team to enable data-driven decision-making and create a culture of sustainment to the organization. EDUCATION AND EXPERIENCE REQUIREMENTS: The qualified candidate should have a minimum of a high school diploma or equivalent. 2 year Associates Degree preferred (three to five years of experience in healthcare or related field may be substituted for required degree). Current experience in healthcare setting and knowledge of regulatory requirements (i.e., The Joint Commission, DOH, IPRO, CMS) is preferred. Excellent written and interpersonal communication skills, demonstrated leadership and facilitation skills are essential for this role. Strong Microsoft Office (Excel, Word, PowerPoint) skills are required.

Responsibilities

  • Conducts outreach to patients identified as having care gaps, coding opportunities or need to be seen by provider and schedule appointments as needed.
  • Prepares project status reports as needed by Primary Care Director.
  • Able to read and understand the quality incentive program incentives and identify areas for improved efficiencies and outcomes.
  • Utilizes all resources available, critical thinking skills and creative approaches to work through obstacles.
  • Learns, understands, and assists with workflows within the EHR as necessary to meet program objectives.
  • Consults with the Director and members of the quality team to understand processes, data collection, report details, and other aspects associated with population health program objectives.
  • Maintains project documentation including resources used, decisions made, outcomes and agreements.
  • Expected to evaluate and monitor reimbursement opportunities and track improvement in revenue as it relates to value-based programs.
  • Responsible for tracking data reporting measures.
  • Responsible for data mining of electronic medical records, gap closure, identification of trends and barriers.
  • Assists department leaders in identifying and addressing operational issues which impact patient satisfaction.
  • Maintains up-to-date knowledge of regional and national benchmarks, survey tools and customer service initiatives.
  • Continuously evaluates initiatives and actively leads solutions.
  • Collaborates with leaders to develop and reinforce metrics for improvement opportunities
  • Supports Ellis Strong initiatives.
  • Manages project time lines.

  • Well-developed ability to assess trends and insight from multiple data sets and large amounts of data, using advanced Excel and other statistical and analytics solutions
  • Detail-oriented and results driven with a passion for producing high-quality deliverables
  • Excellent communication skills and the ability to communicate actionable findings in clear and concise language
  • Strength in multitasking and managing competing demands
