About The Position

Community Health Alliance is looking for full-time Population Health Outreach Specialist. Join our talented team of Population Health Outreach Specialist that focus on serving the community and helping those in need by creating healthy outcomes for patients of every income. We operate six convenient locations throughout Reno and Sparks, providing comprehensive, top-quality medical services, dental and pediatric care, behavioral health, low-cost pharmacies, nourishing food pantries, and more. Job Summary/Objective The Population Health Outreach Specialist's primary objective is to work with the Population Health team on value-based program outreach projects. This role is dedicated to strategic outreach efforts prioritized to address screening/preventative gap closure needs which align with value-based partnerships. It involves organizing, engaging, and tracking outreach efforts that promote health and wellness in the community. This role manages the workflow and support of primary care provider (PCP) assignment/reassignment with health plan partners. This position must demonstrate exceptional customer service skills and professionalism. This position is required to be proficient in reading/writing/speaking Spanish. Requirements

  • Exceptional customer service skills, patient, and professionalism

  • Knowledge of Electronic Medical Records (EMR) system scheduling and patient chart navigation or ability to quickly learn
  • Demonstrates experience managing tracking reports (Excel spreadsheets)
  • Skilled with Excel and Microsoft Office
  • Ability to summarize reporting information
  • Knowledge of medical terminology
  • Ability to work and interact effectively and positively with other staff members to build and enhance teamwork
  • Skill in accuracy, attention to detail, and organization
  • Ability to maintain confidentiality of financial and medical information according to Health Insurance Portability and Accountability Act (HIPAA) regulations
  • Ability to solve practical problems and identify pathways to resolve risks and barriers
  • Ability to exercise good judgment in appraising situations and making decisions
  • Ability to understand and respond appropriately, effectively and sensitively to any population
  • Ability to read, write legibly and calculate mathematical figures
  • Ability to hear and speak to converse over the telephone and in-person in a professional and respectful manner, in both English and Spanish
  • Can operate various office equipment, such as calculator, copy machine, fax, smart phone, and telephone system
  • High School Diploma or GED required.
  • Must be able to read, write, and speak Spanish fluently.
  • Ability to maintain driver's license and to travel throughout the community as well as between clinic sites as needed.
  • Ability to sit or stand.
  • Ability to express or exchange ideas by means of spoken word to impart oral information to co-workers or patients and to convey spoken instructions to others accurately, loudly or quickly.
  • Ability to hear and understand verbal communication.

Nice To Haves

  • One year of relevant experience is preferred.
  • Experience with healthcare related outreach calls, managing/tracking healthcare related data, and/or experience with an Electronic Medical Records (EMR) system preferred.
