Job Summary/Objective The Population Health Outreach Specialist's primary objective is to work with the Population Health team on value-based program outreach projects. This role is dedicated to strategic outreach efforts prioritized to address screening/preventative gap closure needs which align with value-based partnerships. It involves organizing, engaging, and tracking outreach efforts that promote health and wellness in the community. This role manages the workflow and support of primary care provider (PCP) assignment/reassignment with health plan partners. This position must demonstrate exceptional customer service skills and professionalism. This position is required to be proficient in reading/writing/speaking Spanish.
