About The Position

Community Health Alliance is looking for a Population Health Medical Assistant. Join our talented Medical team that focuses on serving the community and helping those in need by creating healthy outcomes for patients of every income. We operate six convenient locations throughout Reno and Sparks, providing comprehensive, top-quality medical services, dental and pediatric care, behavioral health, low-cost pharmacies, nourishing food pantries, and more. Job Summary/Objective The Population Health Medical Assistant supports Community Health Alliance's clinic operations and Population Health initiatives. This position performs patient outreach, manages transitions of care coordination efforts, supports data collection, and manages reporting related to HEDIS metrics. This role enhances care quality by improving gap closure rates such as hemoglobin A1c, preventive screenings, and medication adherence. The role includes maintaining health data dashboards, appointment preparation, and hospital discharge follow-up.

Requirements

  • Working knowledge of HEDIS measures, NCQA standards, and quality improvement processes.
  • Proficient in CPT/ICD-10 coding and documentation standards.
  • Strong Electronic Health Records (EHR) navigation and data extraction capabilities; experience with chart scrubbing and appointment prep.
  • Ability to independently manage data using Excel, reporting tools, and project management platforms.
  • Understanding of value-based care contracts and health plan compliance requirements (Medicaid, Medicare, Commercial).
  • Proficiency in Microsoft Office (Excel, PowerPoint, Word) and comfort using collaborative tools.
  • Strong communication, problem-solving, and critical thinking skills.
  • Certification as a Medical Assistant required; experience in a Federally Qualified Health Center (FQHC) is preferred.
  • Minimum of two years of medical assisting experience required.
  • Experience with Electronic Health Records (EHR) required.
  • Spanish language fluency required.
