About The Position

The Population Health Analyst will support Bridges Health Partners (Bridges) Clinically Integrated Network (CIN) and Accountable Care Organization (ACO). This role is instrumental to evaluate the organization's performance under various value-based contracts and to support initiatives to improve quality, reduce cost, and optimize health outcomes. This is a hybrid position, primarily working remotely from home and on-site at the assigned hospital in Western Pennsylvania, with occasional visits to Bridges’ Corporate Offices in Warrendale, PA.

Requirements

  • Advanced software skills including Microsoft Excel, Word, and PowerPoint.
  • Excel skills include: pivot tables, linked spreadsheets, lookup formulas, and visualizations.

  • Bachelor’s degree in Data Analytics, Data Science, Population Health, Accounting, Finance, Health Services Administration, Healthcare Administration, Economics, Statistics, Public Health, Health Outcomes Research, or related field of study.
  • 2 years of experience involving data analytics, data visualizations, data warehousing, and presenting to key stakeholders.

Nice To Haves

  • SQL experience – including working with large data sets, writing queries, data cleansing, troubleshooting, and reporting.
  • Basic understanding of statistical terminology and methods.
  • Experience calculating and evaluating population health measures/metrics.
  • Prior experience working with value-based contracts and risk-based arrangements.
  • Background in analyzing claims, cost/utilization data, and quality measures.
  • Understanding of CMS ACO programs, HCC risk adjustment, HEDIS measures, quality improvement methodologies, and care management workflows.
  • Prior experience working in a clinically integrated or accountable care environment either in a provider or health plan setting.
  • Master’s degree (preferred) in Data Analytics, Business Analytics, Health Services Administration, Business Administration, or graduate degree in a related field of study.
  • 1 year experience in healthcare such as health plan, healthcare system or large, multi-specialty physician group or IPA, or related healthcare field – preferred but not required.

Responsibilities

  • Utilize knowledge of business needs and customer needs, analytic techniques, data visualization, and presentation skills to analyze and interpret clinical, claim, and population health data to support Bridges strategic and operational goals.
  • Support the design and evaluation of interventions to improve quality scores, reduce avoidable utilization, and optimize value-based performance.
  • Use Bridges’ claim data warehouse, population health tools, electronic health record, health plan population health tools, and other information systems to provide clinical and claims-related reporting and informatics support for members of Bridges and for the management of Bridges’ value-based and at-risk contracts.
  • Build dashboards and automated reports (Tableau/Excel/etc.) to track metrics and performance.
  • Provide analysis and interpretation of Bridges performance in value-based and at-risk contracts to support clinical redesign across a clinically integrated network.
  • Translate complex data into clear recommendations for clinical, operational, and executive stakeholders.
  • Support the design and evaluation of interventions to improve and optimize value-based performance.
  • Provide insights to reduce out-of-network migration and improve network utilization.
  • Collaborate with Bridges leadership, clinical teams, and ACO/CIN partners.
  • Participate in ACO committees and workgroups, presenting data and recommendations as needed.
  • Act as the primary point of contact for data exchange and reporting for the assigned PHO/hospital, ensuring timely delivery of insights to hospital leadership and clinical teams.
