FTE: .60, Shift: Days, Schedule: M-F Position Summary The Access physician provides comprehensive outpatient medical care in collaboration with the primary care practices within a region. This role is designed to offer in person clinical care, ensuring continuity of care in collaboration with virtual care providers and support primary care practices during provider absences, high patient volumes, or staffing shortages. The Access physician adapts quickly to new clinical environments and collaborates with multidisciplinary teams to deliver high-quality, patient-centered care and works collaboratively within a multidisciplinary team.

Requirements

  • Board certified/eligible
  • Licensed in the State of Washington by the time of employment
  • DEA and state prescriptive authority
  • Current BLS for Healthcare Providers certification by the American Heart Association

Responsibilities

  • Delivers primary care services, including preventive care, chronic disease management, acute illness evaluation, and health screenings, to a diverse patient population
  • Diagnoses and treats acute and chronic conditions using evidence-based guidelines
  • Provides in-person management of patients as needed from the virtual care team
  • Provides care for the primary care practice for clinicians who are away or when the clinics experience increased demand
  • Refers to specialists when necessary
  • Timely and accurate documentation, inclusive of direct patient encounters and asynchronous care
  • Collaboration with a multidisciplinary team including advanced practice providers, RNs, pharmacists and care coordinators/community health workers
  • Supports transitions of care and reduced re-admissions and ED use
  • Proactively participates in quality improvement initiatives to enhance care delivery and care outcomes
  • Provides clinical guidance and supervision to clinical and nonclinical team members
  • Participates in team meetings as requested

  • Travels between clinic sites as requested
