Requirements

  • Active, unencumbered NP or PA license in the state you will be practicing
  • Minimum of one year of clinical experience (geriatrics or primary care preferred)
  • Compassionate, patient-centered approach and strong clinical judgment
  • Valid driver’s license and reliable transportation

Responsibilities

  • Deliver personalized, high-quality care to older adults in homes and senior living settings.
  • Conduct comprehensive assessments and develop individualized care plans.
  • Manage chronic conditions with skill and empathy.
  • Collaborate across clinical disciplines (primary, psych, palliative, neuro) for whole-person care.
  • Build trusted relationships with residents, caregivers, and community partners.
  • Maintain thorough documentation and consistent communication.
