As a Physician Assistant or Nurse Practitioner, you’ll join a purpose-driven team that: Delivers continuum-based care – integrating primary, psych, palliative, and home-based services. What Makes this Role Unique Continuum-Based Care (Clinical Excellence): Deliver high-quality care across home, senior living, and community settings, support by a multidisciplinary team in palliative, psych, and neurology. Dementia Leadership: Build deep expertise in dementia-informed care and caregiver support as part of our expanding geri-psych and cognitive health initiative. Requirements

  • Active, unencumbered NP or PA license in the state you will be practicing
  • Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred)
  • Compassionate, patient-centered approach and strong clinical judgment
  • Valid driver’s license and reliable transportation

Responsibilities

  • Delivering personalized high-quality care to seniors in their homes, assisted living, hospice and nursing facilities.
  • Delivering dementia-informed, relationship-based care across the continuum of care.
  • Conducting comprehensive assessments and developing tailored care plans for older adults.
  • Managing chronic conditions and performing care with skill and empathy for the patients and their families.
  • Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.
  • Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
  • Maintaining consistency in documentation, follow-through, and patient communication.
