Program Summary Beth Israel Lahey Health Primary Care's Extended Care Community Program (ECCP) provides patients with personalized, high quality medical care within rehabilitation and long-term facilities. We provide patients with the essential medical care during their stay and support them in transitioning back to their primary care physician upon discharge home or to another appropriate care setting. ECCP physicians and nurse practitioners are board-certified seasoned caregivers who are committed to working with patients, families, as well as the facilities' team members, to provide comprehensive and coordinated patient care. This role is an employment model position in the Harvard Medical Faculty Physicians at Beth Israel Deaconess Medical Center, Inc. (HMFP) Job Summary: Reports to the Extended Care Community Program’s medical director. The primary care physician role for the Extended Care Community Program is responsible for providing medical care services at skilled nursing facilities, rest homes, assisted livings and home settings. The physician will be responsible with working collaboratively with ECCP nurse practitioner(s) and interdisciplinary teams at the skilled nursing facilities.
