About The Position

Program Summary Beth Israel Lahey Health Primary Care's Extended Care Community Program (ECCP) provides patients with personalized, high quality medical care within rehabilitation and long-term facilities. We provide patients with the essential medical care during their stay and support them in transitioning back to their primary care physician upon discharge home or to another appropriate care setting. ECCP physicians and nurse practitioners are board-certified seasoned caregivers who are committed to working with patients, families, as well as the facilities' team members, to provide comprehensive and coordinated patient care. This role is an employment model position in the Harvard Medical Faculty Physicians at Beth Israel Deaconess Medical Center, Inc. (HMFP) Job Summary: Reports to the Extended Care Community Program’s medical director. The primary care physician role for the Extended Care Community Program is responsible for providing medical care services at skilled nursing facilities, rest homes, assisted livings and home settings. The physician will be responsible with working collaboratively with ECCP nurse practitioner(s) and interdisciplinary teams at the skilled nursing facilities. Requirements

  • Graduate degree from an accredited School of Medicine and Residency Program.
  • Qualified candidates should be board-certified/board-eligible in internal medicine or family medicine with a focus on geriatric medicine.
  • Must possess a valid medical license.
  • Demonstrated ability to use medical record systems to thoroughly and properly document patient visits.

Responsibilities

  • Must be familiar with applicable state and federal regulations, i.e. CMS, JCAHO, related to care in skilled nursing facility.
  • Understanding of systems of care delivery, environment and culture in the skilled nursing facility.
  • Assist facilities in maintaining regulatory compliance and ensure that services provided are consistent with current standards of medical practice.
  • Work collaboratively with all care teams, nursing, rehabilitation specialists, pharmacists, hospices in the care management of skilled nursing patients.
  • Provide after-hours backup phone support to ECCP on-call nurse practitioners.
  • Participate in departmental quarterly meetings by participating in presentations, case studies.
  • Participate in peer review, quality assurance, provider meetings, family meetings and other clinical meetings.
  • Obtain health history and perform physical examinations utilizing medical instruments and equipment.
  • Order or execute various tests, analyses, and diagnostic images to provide information on a patient’s condition.
  • Analyze reports and findings of tests and examinations, and diagnose condition of patient.
  • Accurately diagnose and treat a wide-range of acute and chronic diseases and injuries
  • Administer or prescribe treatments and medications appropriate to a patient’s diagnosis.
  • Prescribe medications in accordance with state and federal statutes and professional practice guidelines.
  • Promote health by advising patients concerning diet, hygiene, and methods for prevention of disease.
  • Prepare records and reports as required in support of services rendered in accordance with established procedures.
  • Counsel patient and, if appropriate, family on diagnosis and treatment plans to ensure understanding and follow-through.
  • Make referrals to specialists when appropriate for a patient's condition and treatment.
  • Support BIDHC culture and promote team-based delivery of patient care.
