Nurse Practitioner – Concierge Geriatric Care (Palm Beach / Boca Raton) Full-Time | In-Home + Telehealth A concierge-style geriatric practice in the Palm Beach and Boca Raton area is seeking a Nurse Practitioner to provide high-quality, individualized care for a small, dedicated patient panel. Requirements
- Active Florida Nurse Practitioner license
- Experience or strong interest in geriatrics
- Comfortable conducting in-home visits throughout Palm Beach and Boca Raton
- Organized, professional, and confident using technology/telehealth tools
Responsibilities
- Conduct comprehensive in-home geriatric assessments
- Provide ongoing care and follow-up visits through home appointments and telehealth
- Collaborate with the physician team as needed while maintaining a high level of independence
- Support care planning, medication management, and chronic disease oversight
- Maintain accurate documentation and utilize EMR/telehealth tools
