About The Position

The Physical Therapist II is responsible for providing evaluation and treatment of patients within legal and professional guidelines, and hospital and departmental policies and procedures. This role involves instructing and directing Physical Therapist Assistants and Rehab Technicians. The Physical Therapist II evaluates and implements plans of care to improve, restore, and enhance physical functional performance, and promote injury prevention. Additionally, the position requires performing documentation and quality improvement as outlined in departmental policies and procedures. Requirements

  • Bachelor's Degree Physical Therapy
  • 1 year Physical Therapist
  • Cardiopulmonary Resuscitation (CPR) - American Heart Association
  • Certified as required by specialization
  • Physical Therapist (PT) - State Licensing Board

Nice To Haves

  • Master's Degree Physical Therapy Preferred
  • Doctorate Physical Therapy

Responsibilities

  • Providing evaluation and treatment of the patient within the parameters of legal and professional guidelines, and hospital and departmental policies and procedures
  • Instructs and directs Physical Therapist Assistants and Rehab Technicians as assigned
  • Evaluates and implements the plan of care to improve, restore and enhance physical functional performance, and promote injury prevention
  • Performs documentation and quality improvement, as outlined in departmental policies and procedures
  • Establish a plan of care that is realistic and has measurable goals
  • Communicate and involve other disciplines, physician, family and patient in developing care plan
  • Provide those services or treatments requiring physical therapist skill, according to the written patient care plan and the signed plan of treatment
  • Demonstrate understanding of correlation between OASIS assessments and outcomes reporting
  • Communicate regularly with all disciplines involved in the care of the patient
  • Complete and initial and ongoing assessment (OASIS if appropriate) to determine level of functioning
