About The Position
Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. Lymphedema Physical Therapist: Are you a compassionate and skilled Physical Therapist with experience in lymphedema management? We’re looking for a dedicated clinician to join our team and provide specialized care to patients in need of lymphedema treatment and education. Requirements
- A graduate of an accredited Physical Therapy program with a valid state license.
- A passion for working with patients and a commitment to delivering high-quality, patient-centered care.
- Exceptional communication and interpersonal skills, with the ability to connect with diverse patient populations.
- A team player who fosters a collaborative environment and embraces professional development.
- A strong desire to make a real difference in the lives of others, inspiring and motivating patients on their journey to recovery.
Responsibilities
- Evaluate and treat patients with lymphedema and related conditions
- Develop individualized care plans using evidence-based practices
- Perform Complete Decongestive Therapy (CDT), including:
- Manual Lymphatic Drainage (MLD)
- Compression bandaging and garment fitting
- Therapeutic exercise
- Skin care education
- Educate patients and caregivers on self-management techniques
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with interdisciplinary teams including nursing, OT, and physicians
- Maintain accurate and timely documentation in accordance with regulatory standards
