About The Position

Physical Therapist Assistant (PTA) San Joaquin Nursing and Rehabilitation Pay Range: $40–$45 per hour (Based on Experience) San Joaquin Nursing and Rehabilitation is seeking a motivated and compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. Position Summary The Physical Therapist Assistant (PTA) provides direct patient care under the supervision of a licensed Physical Therapist. The PTA implements treatment plans, documents patient progress, and collaborates with the interdisciplinary team to ensure optimal outcomes for residents.

Requirements

  • Graduate of an accredited Physical Therapist Assistant program.
  • Current, active, and unencumbered PTA license in the State of California.
  • Strong understanding of physical therapy principles and rehabilitation techniques.
  • Ability to follow treatment plans and adapt interventions based on resident response.
  • Excellent documentation and time management skills.
  • Effective communication and teamwork abilities.

Nice To Haves

  • Experience in skilled nursing or post-acute setting preferred.
  • Proficiency with electronic medical records; experience with PointClickCare (PCC) preferred.

Responsibilities

  • Provide physical therapy treatments as directed by the supervising Physical Therapist.
  • Implement treatment plans established by the Physical Therapist and physician.
  • Document daily treatment notes and weekly progress notes in accordance with state practice acts and payer requirements.
  • Communicate resident progress, concerns, and updates to the supervising Physical Therapist and interdisciplinary team.
  • Assist nursing department with training of Restorative Aides as appropriate.
  • Participate in care plan meetings, Patient Care Conferences, and interdisciplinary team meetings as needed.
  • Educate residents and families on exercise programs, safety precautions, and maintenance programs.
  • Assist in discharge planning and caregiver training.
  • Maintain therapy equipment and ensure treatment areas are clean and organized.
  • Report equipment issues promptly to ensure safe and effective operation.
  • Maintain professional, respectful communication with residents, families, and staff.
