About The Position

A Home Health Physical Therapist provides skilled rehabilitation services in patients’ homes to improve mobility, strength, balance, and functional independence. This role focuses on delivering individualized, patient-centered care in a home setting to support recovery, prevent decline, and enhance quality of life. The objective of the role is to improve patients’ independence, safety, and overall function by delivering high-quality physical therapy services directly in the home setting.

Requirements

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
• Current state Physical Therapist licensure
• CPR certification (or ability to obtain)
• Valid driver’s license and reliable transportation
• Strong clinical reasoning and ability to work independently
• Excellent communication and patient education skills
• Ability to adapt treatment approaches to varied home environments
• Strong organizational and time management skills
• Compassionate, patient-centered approach to care

Nice To Haves

• Prior home health or outpatient experience preferred

Responsibilities

• Perform in-home physical therapy evaluations and functional assessments
• Develop and implement individualized treatment plans based on patient needs and home environment
• Provide skilled interventions including therapeutic exercise, gait training, balance training, and functional mobility training
• Assess home safety risks and recommend adaptive equipment or environmental modifications
• Educate patients and caregivers on safe mobility, fall prevention, and home exercise programs
• Monitor patient progress and adjust treatment plans as needed
• Document patient care accurately and in a timely manner according to agency and regulatory standards
• Collaborate with physicians, nurses, and other members of the healthcare team
• Travel to patient homes within assigned geographic area
