Requirements

Graduate of accredited physical therapy program
Continuing education as required by state licensing boards
MN or WI Physical Therapy (PT) License upon start date
Must provide proof of MN and WI PT license within 4 months of start date, if both MN and WI PT licenses are not present on start date
Must have a vehicle with auto insurance and a valid driver’s license and meet all criteria required under Lakeview’s Automobile Liability Guidelines
Current BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
Ability to read, write, and speak the English Language
