About The Position

A Home Health Physical Therapist delivers rehabilitation services directly in patients’ homes, helping individuals regain strength, mobility, and independence after illness, injury, surgery, or due to chronic conditions. This role emphasizes individualized care in real-world living environments and requires strong clinical skills, independence, and flexibility. The goal of the role is to improve patients’ functional independence, safety, and quality of life by delivering high-quality, personalized physical therapy in the home setting.

Requirements

• Doctor of Physical Therapy (DPT) or equivalent degree
• Active Physical Therapist license in the state of practice
• Current CPR certification (or ability to obtain)
• Valid driver’s license and dependable transportation
• Strong clinical judgment and ability to work independently
• Excellent communication with patients, families, and healthcare teams
• Time management and organizational skills for managing multiple cases
• Adaptability to diverse home environments and patient conditions
• Compassionate, patient-centered approach to care

Nice To Haves

• Prior home health experience preferred

Responsibilities

• Perform in-home physical therapy evaluations and reassessments
• Develop and implement personalized treatment plans based on patient needs and home environment
• Provide hands-on therapy, therapeutic exercise programs, gait and balance training, and functional mobility work
• Assess safety risks in the home and recommend adaptive equipment or modifications
• Educate patients and caregivers on safe movement, recovery strategies, and home exercise programs
• Track patient progress and adjust care plans accordingly
• Maintain timely, accurate documentation in compliance with clinical and regulatory standards
• Collaborate with physicians, nurses, occupational therapists, and other care team members
• Travel to patient residences within assigned service areas
