Requirements

Pharm.D. (doctorate degree in pharmacy) or equivalent experience.
Licensed (or eligible for licensure) to practice pharmacy in the State of Washington at initiation of Residency program.
Licensed in the State of Washington as registered Pharmacist by September 1st of current Residency year. If there is a problem with licensing an exception can be made to get a 60-day extension.
Must possess basic computer skills related to Windows navigation, mouse usage, accurate keyboarding, email.

Responsibilities

In collaboration with the health care team, provide safe and effective patient care to a diverse range of patients, including those with multiple co-morbidities, high-risk medication regiments, and multiple medications following a consistent patient care process.
Ensure continuity of care during patient transitions between care settings.
Demonstrate ability to manage formulary and medication-use process, as applicable to the organization.
Prepare, dispense, and manage medications to support safe and effective drug therapy for patients.
Demonstrate ability to evaluate and investigate practice, review data, and assimilate scientific evidence to improve patient care and/or the medication use system.
Demonstrate leadership and management skills.
Provide effective medication and practice-related education to patients, caregivers, health care professionals, students, and the public.
Effectively employs appropriate preceptors’ roles when engaged in teaching.
Maintain and comply with all sections of the Central Washington Hospital PGY1 Pharmacy Residency Resident Manual.
Required to maintain active Basic Life Support (CPR) certification.
Other duties as assigned.
