Requirements

  • Pharm.D. (doctorate degree in pharmacy) or equivalent experience.
  • Licensed (or eligible for licensure) to practice pharmacy in the State of Washington at initiation of Residency program.
  • Licensed in the State of Washington as registered Pharmacist by September 1st of current Residency year. If there is a problem with licensing an exception can be made to get a 60-day extension.
  • Must possess basic computer skills related to Windows navigation, mouse usage, accurate keyboarding, email.

Responsibilities

  • In collaboration with the health care team, provide safe and effective patient care to a diverse range of patients, including those with multiple co-morbidities, high-risk medication regiments, and multiple medications following a consistent patient care process.
  • Ensure continuity of care during patient transitions between care settings.
  • Demonstrate ability to manage formulary and medication-use process, as applicable to the organization.
  • Prepare, dispense, and manage medications to support safe and effective drug therapy for patients.
  • Demonstrate ability to evaluate and investigate practice, review data, and assimilate scientific evidence to improve patient care and/or the medication use system.
  • Demonstrate leadership and management skills.
  • Provide effective medication and practice-related education to patients, caregivers, health care professionals, students, and the public.
  • Effectively employs appropriate preceptors’ roles when engaged in teaching.
  • Maintain and comply with all sections of the Central Washington Hospital PGY1 Pharmacy Residency Resident Manual.
  • Required to maintain active Basic Life Support (CPR) certification.
  • Other duties as assigned.
  • Demonstrate Standards of Behavior and adhere to the Code of Conduct in all aspects of job performance at all times.
