Works from July 1 each year until June 30th of the following year. Works on a team with other clinical pharmacists. Primary responsibilities include the provision of pharmaceutical care services. Provides therapeutic drug monitoring, dosing consultation, and the clinical evaluation of medication regimens. Stresses the reduction and elimination of adverse drug events. Counsels patients and family members on appropriate medication use. Participates in team meetings and rounds. Maintains an active practice in an area of interest, and serves as a resource for drug information to physicians, pharmacists, nurses, healthcare professionals, and the public. Assists in formulary decisions. Precepts pharmacy students. Present residency project, dispenses medications as needed, and helps in the maintenance of inventory, records, files and references.
