About The Position

Under the supervision of the local ambulatory pharmacy management team, the PGY2 Pharmacy Resident - Ambulatory Care is an advanced pharmacy residency role that will build upon previous knowledge gained in a pre-requisite PGY1 Pharmacy Residency role and is responsible for providing patient care and advanced clinical pharmacy services in the ambulatory care pharmacy space, including referral based HOPD pharmacist managed clinics, specialty pharmacy, and population health pharmacy services. This role involves collaborating with prescribers in the ambulatory setting and providing direct patient care managing medication therapy. The PGY2 Pharmacy Resident - Ambulatory Care ensures effective communication with prescribers regarding identified medication related problems and potential solutions and documents patient interactions across various electronic medical record (EMR) platforms.

Requirements

  • Current state Registered Pharmacist License
  • If not active at time of hire, license is required by September 1st of residency year
  • B.S. in Pharmacy or PharmD degree required.
  • Participation in, or previous completion, of an ASHP accredited PGY-1 Pharmacy Practice Residency Program
  • Certificate of Completion to be provided to the Residency Program Director (RPD) or designee no later than 30 days from the start of the residency year
