Summary: The Medication Safety Officer (MSO) provides systemwide leadership and strategic oversight for the comprehensive medication safety plan across our integrated delivery network, encompassing hospitals and ambulatory settings. The MSO collaborates with all care disciplines, including pharmacy, nursing, medical, quality, laboratory and risk management teams to identify and mitigate medication-related risks, standardize care practices, and foster a culture of safety across all sites of care. Using data-driven analysis and systemwide process improvement, the MSO drives adoption of best practices, optimizes medication-use technology, ensures regulatory compliance, and aligns medication safety initiatives with organizational system goals under the direction of the Vice President of Quality and Patient Safety and Chief Pharmacy Officer.

Requirements

  • Doctorate: Pharmacy (Required)
  • Pharmacist License (PS LICENSE) - State of Florida (FL)
  • Complexity of Work: Must be knowledgeable in many aspects of pharmacy practice with the ability to know and understand the safety implication in a variety of settings. Must be able to work within various disciplines, possess excellent leadership, and interpersonal skills and the ability to establish and maintain effective working relationships with all staff levels of the organization including physicians. Effective verbal and written communication, problem solving and computer skills. Must have exceptional program development knowledge and skills.
  • Five (5) years of hospital pharmacy experience.

Responsibilities

  • Medication Safety Program Leadership: Strategically plans, develops, implements and oversees the Memorial Healthcare System (MHS) Medication Safety Plan to ensure alignment with organizational goals, accreditation standards, and state/federal regulations. Proactively develops, leads and evaluates error-prevention strategies based on regulatory standards, best practice guidelines, recommendations, published literature, and external /internal data sources. Leads systemwide error prevention strategies and continuous quality improvement initiatives related to medication safety
  • Policy, Process and Technology Integration: Collaborates and supports clinical teams and informatics to proactively integrate medication safety considerations in the design and implementation of medication-use policies and processes (i.e., selection and procurement, dispensing, administering, monitoring, evaluating). Provides medication safety expertise for technology implementations, formulary management, workflow process design, and clinical decision support.
  • Event and Data Management: Assists in investigation and analysis of medication errors, near misses, and adverse drug events (ADEs). Performs root cause analyses (RCA) as well as failure mode and effects analyses (FMEA). Conducts and supports site assessments and observational audits to evaluate the medication-use process and adherence with established expectations. As part of the medication safety plan, tracks, trends, analyzes and presents relevant audit, medication safety data and key performance indicators identifying and mitigating risks proactively for the medication-use process. Leads systemwide working forum on medication safety.
  • Education and Culture of Safety: Promotes a "Just Culture" approach to error reporting, surveillance and learning. Serves as a resource and subject matter expert to clinical teams and leadership on medication/patient safety and quality standards related to medication management to provide ongoing accreditation and regulatory readiness. Works collaboratively with clinical teams on error investigation and classification according to national standards. Promotes and participates in academic efforts. Develops and delivers education and competency programs related to medication safety practices, including oversight and management of Risk Mitigation Strategies (REMS) .
