Carilion New River Valley Medical Center is seeking a pharmacist to manage and coordinate the pharmacy's efforts to support our outpatient Infusion Center. Potential work schedules are 6-hour shifts Monday through Friday or three 8-hour shifts and one 6-hour shift Monday through Thursday. However, pharmacy leadership may collaborate with the chosen candidate to develop a schedule that meets both the hospital's operational needs and the needs of the individual. Some of the pharmacist's duties may include: Coordinating receipt, storage, and administration of "white bag" and "free drug" medications Communicating with specialty pharmacies and payers as needed to address delays or discrepancies Managing inventory of infusion medications Verifying infusion medication orders in Epic Ensuring proper charging for non-formulary infusion medications Entering, reviewing, and optimizing Epic Therapy Plans Collaborating with nurses and providers to clarify infusion orders Working closely with Infusion Center nursing staff and leadership to develop and perfect workflows to promote patient safety and throughput Assisting with the provision of investigational drugs at the CNRV Infusion Center Educating other pharmacists on infusion center-related processes and workflows Depending on candidate interest, cross-training may be provided in other clinical areas such as the Emergency Department and night shift. The Pharmacist Participates as an active member of interdisciplinary care teams Advises on the most efficacious treatments and participates in pharmacotherapeutic decision making. Participates in collaborative practice protocols. Provides virtual pharmacy consults to Carilion Clinic Ambulatory Offices. Assists with drug information and evidence-based medicine. Consults and provides healthcare providers advice on medications. May need to respond to patient emergencies. Reviews and verifies medication orders for accuracy. Assures preparation of sterile/nonsterile compounding according to Standard Operating Procedures. Assures delivery of medications in a timely manner. Provides education and training for healthcare providers (pharmacy, nursing and medical), students, residents, patients, and patient families. Provides feedback to pharmacy students/residents. Maintains competency in all technologies utilized by the Department of Pharmacy. Supports clinical pharmacy initiatives of the institution. Maintains compliance with statutes, laws, regulations, standards and policies governing the practice of pharmacy. Facilitates age specific pharmaceutical care to patients.
