Pharmacist I Full-Time I Third Shift Hospital Pharmacy I Lexington, KY $15,000 Sign on Bonus for Qualified Individuals Department of Pharmacy Services of Baptist Health Lexington provides pharmaceutical services to patients across the continuum of care. These services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation, and utilization of automated dispensing cabinets. Description: Serves as a drug information expert and resource regarding medication management. Provides comprehensive decentralized or pharmacy clinical services including, but not limited to, anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support services. Participates in antimicrobial stewardship and opioid stewardship. Provides medication counseling to patients. Works with inter-professional patient care teams to help patients with transitions to care, including=but not limited to helping prepare patients for discharge. Serves as an active member of the inter-professional team on patient care units. Participates in unitbased huddles and activities if available. Knowledgeable in unit’s metrics, goals, and works with staff on unit to help achieve goals. Interprets and reviews medication orders for correct drug, dosage, route, frequency, allergies, therapeutic duplication, interactions, contraindications, and potential adverse events for all ages served. Ensure drugs are available to patients at the time they are needed to avoid delays in therapy and promote positive patient outcome. Oversees dispensing and reconciliation of medications including controlled substances, management of expiration dates and drug recalls. Performs medication reconciliation for patients. Provides technician oversight and assumes the role of supervisor in routine daily activities. Supervises/performs production of compounded products (sterile and nonsterile, hazardous and nonhazardous) for first doses, scheduled deliveries, and batched products. Demonstrates supervisory skills necessary to plan, organize, and direct activities in the pharmacy in a cost-effective manner. Assists in the orientation and training of pharmacists, technicians, students, and residents; aids in performing competencies on staff (new hire and annual) as requested by manager/director. Acts as preceptor for both students and residents as applicable Maintains knowledge of local, state, and federal regulations regarding pharmacy practice as well as knowledge of all applicable accrediting bodies medication management standards (e.g., the Joint Commission, HFAP, etc.). Effectively utilizes pharmaceutical references. Promotes formulary therapies to the medical staff, contacting providers with alternatives and using knowledge of medication formulary, current interchanges, and non-formulary medication processes to make appropriate recommendations.
