Requirements

  • North Carolina Pharmacy Technician Certification required, or ability to obtain from an approved program within 6 months of employment.
  • Current North Carolina driver’s license required.

Nice To Haves

  • An Associates’ degree in human services, health behavior or health related-field is preferred, or equivalent work experience.
  • Experience with computer database software preferred.

Responsibilities

  • Uses an appropriate problem-solving approach to plan activities, events or programs.
  • Assesses culturally diverse community needs through a variety of methods.
  • Identifies resources and plans of action for tailored initiatives for diverse populations.
  • Assists with development and ongoing maintenance of community resource information.
  • Utilizes available resources to work in collaboration with other agencies.
  • Provides services with consideration of the recipient’s needs.
  • Provides for variation in care or program appropriate to language/cultural differences.
  • Provides assistance to target populations in accessing community resources.
  • When appropriate, determines eligibility for programs.
  • Gathers client demographics, health information and other pertinent data.
  • Collaborates with health care providers to ensure that medication needs of the target population are met in a coordinated manner.
  • Maintains competency in office and computer equipment.
  • Provides for appropriate use of supplies.
  • Follows safety and infection control policies.
  • Helps coordinate communications among human service agencies, providers and target populations.
  • Serves as resource for community programs and organizations.
  • Maintains appropriate client/patient flow and provides appropriate information to client/patient.
  • Identifies patient information needed to provide services to patient and coordinates successfully with the clinic intake team to obtain this information.
  • Reports significant data regarding target population and program outcomes to appropriate audiences.
  • Assists in the collection of qualitative and quantitative data regarding target populations, issues and program outcomes
  • Inputs data into The Pharmacy Connection (TPC) and Pioneer Software programs as instructed through training.
