Requirements

  • Bachelor's Degree BSN
  • 5 years experience required, preferably in cardiovascular disease management including ambulatory care. Labor and Delivery experience required for the Perinatal Transition Coordinator.
  • (RN) REGISTERED NURSE Upon Hire and
  • (BLS) BASIC LIFE SUPPORT course accredited by the American Heart Association (AHA), or American Red Cross (ARC) Upon Hire and
  • (ACLS) ADV CARDIAC LIFE SUPPORT course accredited by the American Heart Association (AHA), or American Red Cross (ARC) for the Perinatal Transition Coordinator Upon Hire and
  • (NRP) NEONATAL RESUSCITATION for the Perinatal Transition Coordinator Upon Hire and
  • (AHWONN-EFM) FETAL MONITORING for the Perinatal Transition Coordinator Upon Hire

Responsibilities

  • Receives and provides immediate responses to inquiries and patient referrals. Establishes immediate relationship between referral sources and hospital.
  • Works closely with collaborating facilities to establish and develop relationship with patients and their families. Ultimately responsible for facilitating the transfers.
  • Works closely with collaborating facilities and services lines to facilitate treatment and transition care as appropriate.
  • Direct liaison and clinical link between Outpatient services and all collaborating services lines and facilities.
  • Facilitate transfers and referrals from outside hospitals with understanding this is a destination center and accredited program.
  • Interfaces with hospital departments, ancillary services, social services, and physicians from collaborating facilities to facilitate seamless and safe transition care.
  • Coordinates bed placement and medical transportation as needed.
  • Interfaces with other hospital departments (i.e., infection control, admitting, pastoral care, and other appropriate service lines) and physicians to facilitate admission process.
  • Assists and participates in rounds to build relationship with patients and the care team, but also facilitate care and smooth transition amongst facilities and interdisciplinary team.
  • Assist in obtaining records and clearances needed for treatment and care.
  • Resource to patients, and families, regarding social services, financial assistance, and outside resources available.
  • Reviews referrals and verifies information. Determines appropriateness of referral based on established criteria and assures that all patients‿ work up and resulting reports are provided in accordance with protocols.
  • Establishes & maintains infrastructure to monitor referring physician practice patterns, alerts team to any change in referral patterns.
  • Tracks data regarding intake and referral sources.
  • Active participant in interdisciplinary meetings among all participating facilities and service lines.
  • Coordinates efforts with appropriate service lines and facilities involved in patient care team to provide streamlined, efficient systems of referral management and service recovery.
  • Assist with development and maintenance of patient education material.
  • Serves as resource and helps train staff related to appropriate clinical specialty and program.
  • Responsible for training future/new coordinators.
  • Facilitates meetings amongst all appropriate hospitals, programs, and service lines to ensure program upholds accreditation standards.
  • Development and maintenance of process maps detailing intake and transition process amongst participating entities as well as process for outside referrals.
  • Participates in inpatient rounds and patient care with UT Southwestern team as appropriate.
  • Works closely with inpatient services and team members from participating entities to ensure patients have all necessary discharge and follow up care.
  • Performs other duties as assigned.
