About The Position

EverHome HealthCare is a well-established and highly respected Home Health and Home Care agency serving the Central Puget Sound. We are a doctor owned company with an expert team to support you. The Occupational Therapist works as the team leader with a team comprised of the care companions, Clinical Director, and rehabilitation professionals to assure that all the clients’ needs are being addressed and expertly met. Duties, Responsibilities, & Accountabilities Including but Not Limited To: Provides skilled occupational therapy in accordance with the physician’s plan of care. Provide, coordinate, and oversee skilled intermittent care to clients and include instruction and education of clients and/or caregivers to promote maximal functional status, in the client’s home. Regularly evaluates and re-evaluates client status. Initiates, develops, implements, and revises the client’s plan of care (POC). Addresses all problems identified in the POC. Reassesses the client following a change in condition or discharge from a 24-hour or longer hospital stay, or as ordered by the physician. Provides educational information to the client/family/caregiver concerning knowledge deficits or needs regarding the client’s condition. Communicates with other disciplines involved in the case as warranted. Good interpersonal, oral, and written communication skills Ability to work efficiently under pressure and work independently and take initiative.

Requirements

  • Registered by the American Occupational Therapy Association or successfully completed the educational requirements of occupational therapy and licensed in the state.
  • Minimum of one (1) year of occupational therapy experience required.
  • Current CPR/BLS
  • Reliable transportation, current Driver’s License, and proof of auto insurance
