The Occupational Therapist works as the team leader with a team comprised of the care companions, Clinical Director, and rehabilitation professionals to assure that all the clients’ needs are being addressed and expertly met.

Requirements

• Registered by the American Occupational Therapy Association or successfully completed the educational requirements of occupational therapy and licensed in the state.
• Minimum of one (1) year of occupational therapy experience required.
• Current CPR/BLS
• Reliable transportation, current Driver’s License, and proof of auto insurance
• Good interpersonal, oral, and written communication skills
• Ability to work efficiently under pressure and work independently and take initiative.

Responsibilities

• Provides skilled occupational therapy in accordance with the physician’s plan of care.
• Provide, coordinate, and oversee skilled intermittent care to clients and include instruction and education of clients and/or caregivers to promote maximal functional status, in the client’s home.
• Regularly evaluates and re-evaluates client status.
• Initiates, develops, implements, and revises the client’s plan of care (POC).
• Addresses all problems identified in the POC.
• Reassesses the client following a change in condition or discharge from a 24-hour or longer hospital stay, or as ordered by the physician.
• Provides educational information to the client/family/caregiver concerning knowledge deficits or needs regarding the client’s condition.
• Communicates with other disciplines involved in the case as warranted.
