About The Position

The Peer Recovery Support Specialist provides peer support services to young adults ages 18 to 25, with substance abuse or co-occurring disorders; and care coordination services to youth and young adults ages 16 to 25 with substance use, mental health, or co-occurring disorders. Provides care coordination and works collaboratively across the systems of care to address family needs, improve family engagement in treatment and supports the youth and family unit. Serves as a member of the youth’s treatment team, providing input, family support, and monitoring to promote success for the family within their community.

Requirements

  • SMART Recovery Proficient.
  • Must be able to travel extensively within region.
  • Must be able to lift up to 15 lbs.
  • High School diploma or GED.
  • Must be in recovery for a minimum of 2 years.
  • Must attain/maintain certification in Non-violent Crisis Intervention and CPR/FA during employment.
  • Must have a valid driver’s license.
  • Must be able to apply for PRSS Certification within 6 months of employment.

Nice To Haves

  • 2 years’ experience preferred.
  • Prior training/experience working with children/youth/families preferred.
  • Knowledge of community resources preferred.
  • Organized with ability manage multiple tasks.

Responsibilities

  • Models, promotes, and practices recovery principles in all activities including hope, empowerment, responsibility, and self-determination.
  • Provides care coordination to ensure transitioned aged youth and young adults with severe SUD or SMI/SED and are at risk of suicide are linked to housing, transportation, family planning, recovery services and other resources that enable successful living in the community.
  • Provides peer support and coordinates with youth and family members by: Life skills coaching, Wellness Recovery Action Planning, Recovery Training e.g., budgeting, navigating the system of care, connecting the youth/family with community services and resources.
  • Works with youth/family team and treatment provider to ensure continuity about common goals for treatment and recovery, support to achieve goals, and monitoring and reporting of progress.
  • Maintains regular contact with youth and young adults through regular home visits and phone calls.
  • Supports family to implement child plans introduced through therapy.
  • Provides transportation to services, courts, etc. as needed.
  • Assists in the development of and supports relapse prevention plans.
  • Collaborates with Crisis Response Team and Treatment Programs that provides services to youth and young adults with SUD or severe SMI/SED and family: Provides input into processes and plan development/implementation
  • Provides input and support for youth and young adults involved in court programs.
  • Assists Intervention Specialist with case management and identification of community resources or other supports.
  • Completes screeners or assessments as indicated at designated intervals and includes outcomes in the youth treatment plan and Wellness Recovery Action Plan.
  • Documents all activities per policy.
  • Ability to assess and respond effectively to crisis situations. Participates in on call for youth and families in program that serve youth and young adults with SUD or severe SMI/SED.
  • Meets all supervision and training requirements as established.
  • Adheres to confidentiality, HIPAA, and risk management policies and procedures including but not limited to completing Incident Reports.
  • Utilizes technology provided to perform job efficiently and effectively.
  • Participates in other functional work by participating in related projects, as applicable and directed by Center leadership.
