Provide and execute a plan to reduce health disparities and increase HIV prevention strategies in targeted communities throughout Cook (non-Chicago), DuPage, and Kane Counties and complete required components of the HIV prevention, Youth Experiencing Homelessness, and Youth Homelessness Demonstration Program as assigned. This position is grant-funded and is subject to the availability of funds. Work Location: 650 E.
