About The Position

The Peer Care Coordinator plays a pivotal role in fostering a supportive and inclusive environment by connecting individuals with peer support resources tailored to their unique needs. This position is responsible for developing, implementing, and managing peer support programs that enhance community engagement and promote mental health and well-being. The coordinator will collaborate closely with healthcare providers, community organizations, and participants to ensure seamless access to peer care services. By monitoring program effectiveness and gathering feedback, the Peer Care Coordinator continuously improves service delivery to maximize positive outcomes. Ultimately, this role aims to empower individuals through peer connections, facilitating recovery, resilience, and sustained personal growth.

Requirements

  • Bachelor’s degree in social work, psychology, public health, or a related field.
  • Experience working in peer support, community outreach, or care coordination roles.
  • Strong understanding of mental health, substance use, or chronic illness peer support principles.
  • Excellent communication and interpersonal skills to effectively engage diverse populations.
  • Proficiency in program management and data collection tools.

Nice To Haves

  • Certification as a Peer Support Specialist or equivalent credential.
  • Experience with grant writing and managing funded programs.
  • Familiarity with electronic health records (EHR) and case management software.
  • Bilingual abilities to serve diverse communities.
  • Advanced training in trauma-informed care or motivational interviewing.

Responsibilities

  • Design, coordinate, and oversee peer support programs that align with organizational goals and community needs.
  • Recruit, train, and supervise peer support specialists to ensure high-quality service delivery.
  • Serve as a liaison between participants, healthcare professionals, and community partners to facilitate comprehensive care.
  • Monitor program metrics, collect participant feedback, and prepare reports to evaluate program effectiveness and identify areas for improvement.
  • Organize workshops, support groups, and outreach activities to promote awareness and engagement in peer support services.
