About The Position

The Gender-Affirming Care Intake & Peer Navigation Coordinator is the primary point of contact and ongoing care navigator for pediatric patients and families seeking services at Anchor Health. This hybrid role manages the full intake process—from initial inquiry through first appointment - and provides longitudinal support across medical, surgical, behavioral health, and social care. Grounded in trauma-informed, patient-centered practices and aligned with WPATH SOC-8, the function of this role ensures timely, equitable, and affirming access to care with the primary objective of reducing barriers, supporting care continuity, and enhancing patient preparedness.

Requirements

  • Bachelor’s degree in a related field (or equivalent experience)
  • 2–3 years in patient coordination, intake, or care navigation
  • Moderate to advanced Excel skills
  • Project development and management skills
  • Strong organizational, communication, and patient-centered skills.

Nice To Haves

  • Experience in gender-affirming or LGBTQ+ care
  • familiarity with transgender and gender-diverse communities
  • Epic or EMR experience; insurance navigation knowledge; bilingual (e.g., Spanish).

Responsibilities

  • Serve as first point of contact; manage intake, onboarding, and registration processes, including data collection, eligibility verification, and appointment prioritization.
  • Provide ongoing patient navigation, including non-clinical education on care pathways, surgical readiness, and support services.
  • Conduct proactive outreach for missed visits, care gaps, and post-visit or post-surgical follow-up.
  • Coordinate referrals and external care, track required documentation, and support surgical planning and recovery.
  • Assist patients with insurance navigation, including prior authorizations, appeals, and identifying financial support options.
  • Identify and address barriers to care (e.g., transportation, safety, access) and connect patients to community and material resources.
  • Monitor regional access to care, respond to service disruptions, and support continuity through care transitions.
  • Maintain accurate documentation in Epic and tracking systems; ensure HIPAA compliance and data integrity.
  • Coordinate scheduling across services and locations, ensuring patient readiness.
  • Provide affirming, culturally competent communication and serve as a consistent point of contact.
  • Collaborate with interdisciplinary teams and contribute to workflow improvements and patient education resources.
